Where did the referral for assessment come from?
What was the main reason for referral to secondary mental health care?
Examples:
- Referred by GP – low mood and anxiety affecting functioning.
- Referred to Street Triage by Yorkshire Ambulance Service – suicidal, required further CAS assessment.
- Referred by Community Links worker – odd behaviour, hearing voices.
- Self-referral – suicidal with clear plan.
- Referred by carer (mother) – exacerbation of symptoms, previous diagnosis of psychosis. Not currently with services.
